How is SEMPHN involved?
SEMPHN is proud to be participating in the Greater Choice for At Home Palliative Care project funded by the Australian Department of Health. We are collaborating with public and private providers including GPs and residential aged care facilities (RACF) to improve access to safe, quality, palliative care.
Community Palliative Care visits for General Practice
SEMPHN has partnered with the Southern Metropolitan Region Palliative Care Consortium (SMRPCC) to offer General Practices the opportunity to connect with local community based palliative care providers. Learn more about this program and how you can apply for visits here.
Palliative Care Access to Core Medicines (PCAM) Project
The PCAM project provides palliative care support and assists people who wish to die at home through timely access to essential medicines. Visit our PCAM page for a complete Core Medicines List (CML) and map identifying the location of participating local community pharmacies in south east Melbourne.

Community Palliative Care visits for General Practice
SEMPHN has partnered with the Southern Metropolitan Region Palliative Care Consortium (SMRPCC) to offer General Practices the opportunity to connect with local community based palliative care providers.
Practices can request a visit from a community palliative care team member to provide information, education, and help improve understanding and access to community palliative care services. Topics will be chosen by the GP clinics and can include symptom management, care planning, palliative care conversations, etc. Visits will be arranged by a liaison officer at a time that suits the practice.
To launch this exciting initiative, SEMPHN & SMRPCC are offering an incentive payment of $1,000 (ex GST) per practice to the first nine General Practices to participate in the initiative. PLACES ARE LIMITED and on a 'FIRST IN' BASIS. Further placements will continue without financial incentive.
