$6 million boost for care coordination in South Eastern Melbourne

Over 1,900 patients with chronic conditions throughout South Eastern Melbourne will benefit from a new round of funding to general practices from the South Eastern Melbourne Primary Health Network (SEMPHN).
SEMPHN is pleased to announce 25 general practices will provide care coordination services for patients with chronic conditions over a period of two years.
In total, $6 million has been allocated to this program across eight local government areas in the south east.
SEMPHN's Chief Executive Officer, Elizabeth Deveny said this funding initiative is a positive step towards better health outcomes for patients with chronic disease.
“The additional funding for general practices will give them greater capacity to work with patients, their carers and other health professionals to connect patients to the best possible care.”
Care coordination is a term used to describe working with a patient to develop a comprehensive plan that helps patients take more control of their health and achieve their goals.
Care coordination is for patients with a chronic condition(s), at risk of admission to hospital, or who may have complex needs (which include the social determinants of health). It is a patient-centred approach that involves the timely coordination of health, community and social services to meet a patient’s needs. It is a partnership between the patient, carers and providers.
