More patients can now access care coordination in south eastern Melbourne

SEMPHN is pleased to announce 21 general practices will provide care coordination services for patients with chronic conditions across south eastern Melbourne. 

Care coordination involves working with a patient to develop a plan that helps the patient take more control of their health and achieve their goals.

Care coordination is for patients with a chronic condition(s), at risk of admission to hospital, or who may have complex needs. It is a patient-centred approach that involves the timely coordination of health, community and social services to meet a patient’s needs.

SEMPHN's Chief Operating Officer, Simon Wrigley, said this funding will help general practices to further support their patients.

"We're excited to welcome a new round of general practices into the world of care coordination. When caring for patients, we know that time and dedicated health workers make a world of difference. This funding offers general practices exactly that; the additional time or staff to develop individual, comprehensive, health plans to connect patients to the best care, when and where they need it."
