    Blood Pressure: Hypertension Identification & Diagnosis

    • Purpose: Establish an accurate average BP to confirm or refute a diagnosis of hypertension
    • Duration: 10-14 days
    • User case: Patients with suspected hypertension/high BP readings in the last 18 months, no hypertension diagnosis.

    Diabetes Monitoring:

    • Purpose: Requests home blood glucose readings 2 or 4 times a day (either fasting, pre-lunch, pre-dinner, and pre-bed) to help newly diagnosed type 2 diabetes patients form a good blood glucose monitoring routine and establish an accurate average blood glucose to inform clinical decisions.
    • Duration: 4 weeks
    • User case: Patients with newly diagnosed diabetes type 2.

    Diabetes Monitoring Fasting:

    • Purpose: Requests home fasting blood glucose readings (pre-breakfast) to help newly diagnosed type 2 diabetes patients form a good blood glucose monitoring routine and establish an accurate average blood glucose to inform clinical decisions.
