These Dementia Pathways are an online list of dementia-related services, supports and resources in south eastern Melbourne. The information on this page is for people living with dementia in the following local government areas: Bayside, Cardinia, Casey, Frankston, Glen Eira, Greater Dandenong, Kingston, Monash, Mornington Peninsula, Port Phillip, and Stonnington.

First steps after receiving your dementia diagnosis
Below are some useful steps to take after finding out about your dementia diagnosis.
Step 1: Speak to your GP
- Speak to your GP about treatment and options for managing dementia.
- Ask for dementia resources from your doctor.
- Ask about specialist appointments, allied health, and advanced care planning.

Step 4: Access Government, social and community support
You could access support from My Aged Care, Services Australia, community organisations and your local council.
Medical care to treat dementia
This section provides a list of dementia specific medical services to treat and care for people living with dementia.
Cognitive, Dementia and Memory Services
The Cognitive Decline & Memory Service (CDAMS) can provide detailed assessment, a diagnosis or explanation of memory and thinking problems that are causing you or your family concern. CDAMS clinics do not offer ongoing treatment post-diagnosis, information, advice and referral on to other services can be provided for any ongoing treatment or support needs.

Aged Care Assessment Services (ACAS)
The Aged Care Assessment Services determine your eligibility for subsidised aged care. A two-part assessment process is undertaken to gain an understanding of what services are best suited to your needs. An eligibility check is conducted online or over the phone, followed by an in-person assessment.
