The Victorian Agency for Health Information (VAHI; as a division of the Victorian Department of Health) and South Eastern Melbourne Primary Health Network (SEMPHN) are collaborating on a data linkage pilot project, focused on cardiovascular disease (CVD) outcomes. CVD is a major cause of illness and death in Australia and treating people with CVD in hospital is more costly than treating them in the community. Integrated care across the primary care and hospital sectors is crucial to ensure timely prevention of CVD hospitalisations. Comprehensive, joined-up primary care and hospital data are important in understanding patient pathways through the health system. There is currently an absence of this data in Victoria!
We’re seeking a Health Consumer Advisor to join the project’s Steering Committee.

Time commitment: We kindly request a minimum commitment of one (1) year. The Steering Committee meets three to four (3-4) times per year, with each meeting lasting no more than two (2) hours. The first meeting is likely to be in August 2023.
Meeting location: Meetings will be via video conference, or in person (Melbourne area) with members' agreement.
Payment: You will receive $237 per meeting attendance.
Support: A designated contact person from VAHI will provide support throughout your engagement.
