Expression of Interest: Integrating non-dispensing pharmacists into general practice

In partnership with the Pharmaceutical Society of Australia (PSA), SEMPHN is seeking eight general practices from the Mornington Peninsula, Bayside and Frankston Local Government Areas (LGAs) to join our Pharmacists in General Practice program.

Why is this program needed?

Similar projects throughout Australia have demonstrated that integrating pharmacists into general practice can result in improved practice efficiency and patient care outcomes.

Designed to support existing practices, the program can help to facilitate more integrated patient care, improve patient medication adherence and enhance prescribing practices.

What is involved?

Successful general practices will gain access to an experienced pharmacist (at no cost to the practice) who will work alongside practice staff to provide training and support, tailored to your specific needs.

The project involves a non-dispensing clinical pharmacist working as part of the general practice team for 15 hours per week. The pharmacist will undertake a variety of medication-related activities, which may include:

  • Providing medication education to patients
  • Transition of care management
  • Identifying and resolving medication issues
  • Answering GPs’ and other practice staff’s medication questions
  • Supporting activities that enhance MBS billing
  • Working with other health care professionals when patients transition in and out of hospital or residential aged care facilities to ensure medicine continuation

The project is expected to commence in March 2024 and end in January 2025.
