The Clinical and Referral Guidance Hub (the Hub) is a SEMPHN supported platform designed to provide access to local referral information and free clinical guidance for primary care professionals working in south east Melbourne.
About the Hub
The Hub brings together key resources to support day-to-day clinical practice, including:
- Localised referral information from local public health networks: Bayside Health and Monash Health.
- Access to clinical guidance via Therapeutic Guidelines
- Quick links to Healthdirect and Statewide Referral
Who can access the Hub?
Clinical Guidance
- General practitioners (GPs)
- Nurse Practitioners working in general practice
- GP Registrars
All working within south east Melbourne.
Referral Guidance
Access to localised referral information is available to all primary care professionals working across south east Melbourne.
How to register
To access the Hub, you will need to register your details, including:
- Name
- Workplace details
- AHRPA number (for access to Therapeutic Guidelines)
After you submit your registration, you will be able to access the Hub and explore referral information along with other available features. SEMPHN will review your registration details to confirm your eligibility for the clinical guidance feature (Therapeutic Guidelines). You will receive an email within two business days confirming your access to Therapeutic Guidelines.

You can also log in via the yellow ‘CRGH login’ button located in the top right-hand corner of the SEMPHN website.
Register to access the Hub
Please note that access to the clinical guidance feature (Therapeutic Guidelines) will be granted within two business days once eligibility is confirmed.
