**About the Role:**<br>This opportunity is for two full time Nurse Practitioners, working fixed term until June 2025, to work to provide coordination of comprehensive care of consumers referred to the Primary Health Clinic (PHC). This role includes identification and management of all relevant comorbidities in this complex cohort including drug dependence treatment, blood borne and sexually transmitted diseases, physical, psychological or psychiatric health needs, along with social and legal issues. For complex clients, the Nurse Practitioner forms the core of the complex case planning and management team, with less complex clients being stabilised and referred to other practitioners, with or without opiate pharmacotherapy.<br>**About Us**<br>The Primary Health Clinic sits as part of Monash Health Drug and Alcohol Service is a service within the Mental Health Program of Monash Health committed to providing opportunities for change and growth for people affected by problematic alcohol and drug use, improving their physical and mental health, and reconnecting them to their family and the community. The Primary Health Clinic provides pharmacotherapies, drug overdose response and specialised heath care, basic nursing and mental health support, general dietary advice, as well as other practical supports for people who inject drugs<br>Other Drug and Alcohol Services offered by Monash Health include:<br>- SECADA, which is a single point of contact for a large network of specialist services which supports youth and adults with a range of alcohol and drug concerns including counselling, care and recovery, non residential withdrawal and intake and assessment<br>- Aboriginal Metropolitan Ice Partnership (AMIP)<br>- Primary Health Clinic<br>- Needle and Syringe Program<br>- Southern Dual Diagnosis Service<br>- The Addiction Medicine Unit<br>- Way Out Program - Non-residential group program<br>**Key accountabilities include:**<br>The Pharmacotherapy Nurse Practitioner would be both an extension of existing services and allow new service capacity to be delivered through the Monash Health Primary Health Clinic with a focus on medically, psychiatrically and behaviourally complex clients who use drugs.<br>**About You**<br>To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge as outlined in the position description:<br>The Nurse Practitioner will be a clinical leader and role model for other health service staff, including nursing, allied health and lived/living experience professionals. Ther Nurse Practitioner will have a caseload and will have a role in liaising, education and coordinating other Monash Health staff members to ensure clinical efficacy and appropriateness of care for clients who use drugs.<br>
- Appropriate tertiary qualifications in a discipline such as Nursing, Social Work, Occupational Therapy or Psychology<br>- Current registration as a Nurse Practitioner with AHPRA<br>- Membership of professional nursing bodies/organisations<br>- Post graduate qualification in related field (mental health and/or drug and alcohol studies)<br>- Current Victorian Driver's License<br><br>
